Facilities Usage

Welcome to the Facility Usage Page for Maywood-Melrose Park-Broadview School District 89.
In alignment with District Policy 8:20 – Facility Usage, District 89 permits responsible individuals and organizations to use school facilities, as long as it does not interfere with educational programming. We are proud to support our community by making our facilities available for use during non-instructional hours.

Application Process

  1. Complete the Facility Usage Application Form / Para completar el Formulario de solicitud de uso de instalaciones en español 
  2. Provide Insurance Certificate Requirements and Necessary Permits:

All non-district facility users are required to provide a Certificate of Insurance (COI).

    • General Liability:Minimum of $1,000,000 per occurrence and $2,000,000 aggregate
    • Automobile Liability: $1,000,000 for owned, leased, hired, or non-owned vehicles
    • District 89 must be named as an additional insured
    • Policies must include primary coverage, waiver of subrogation, and no cross-suits exclusion
    • COI must be submitted at least 15 business days before the event to: Gwen Gordon at [email protected] or 708-450-2088.
    • Insurance carriers must be licensed in Illinois and rated A-X or better by A.M.
    • District 89 may modify requirements based on event risk

Additional Guidelines 

    • Indemnification: All users agree to indemnify and hold harmless District 89, its staff, and board members from any claims or liabilities arising from the event.
    • Damage Responsibility: Users are liable for any damage to district property and may be invoiced for the cost of repairs or replacement.
    • Supervision: Adequate adult supervision is always required. The district is not responsible for accidents, injuries, loss, or theft of property.
    • High-Risk Activities: Events involving inflatables, athletic competitions, or amusement rides must be disclosed in advance and may require:
      • Additional insurance
      • District approval
      • On-site staff/security (at the user’s expense)

Emergency Procedures: Users must follow all posted emergency procedures and cooperate with staff and emergency personnel in the event of an incident.

  1. Wait for approval from the Building & Grounds Supervisor
  2. Pay the invoice 7 days before the event date
    • All payments must be made payable to: “Maywood School District 89.”
    • Only money orders or certified bank checks are accepted. No personal checks permitted.
    • Payments must be received by the due date; otherwise, the application will be canceled.
    • No refunds will be issued; however, credit may be applied toward another available date if scheduling permits.
